Short-term rehabilitation support and reablement care

If you’ve been injured or in hospital for a long period, it might take some time to get back into a routine once you get home. You may also have experienced life-altering physical changes, for example if you’ve lost a limb, had a hip replacement, experienced partial sight loss, or are recovering after a stroke. This is where our reablement carers and rehabilitation support workers can help.

What is reablement care?

Reablement care (sometimes called re-enablement care or transition support) is specifically designed to help you adjust to being home again after a long hospital stay, and to regain the skills and confidence you need to live independently. Short-term rehabilitation support may also be needed if you've had a change in your condition and need to adapt to a new way of life, for example, if your mobility needs have changed or you have new sensory impairments.

Unlike domiciliary care, the aim of this service is not for our carers to do things for you, but to support you to do the things you want. Rehabilitation support or reablement care is usually provided on a short term basis, often with the level of care and hours of support decreasing over time as your recovery progresses.

Our reablement carers and rehabilitation support workers can assist with:

  • Post-operative or wound care and/or medication management
  • Physical and occupational therapy exercises
  • Mobility support, including supporting you to attend follow-up appointments
  • Goal setting and motivation to help you regain your independence
  • Daily living tasks, such as helping you prepare meals
  • Co-ordination of healthcare services
  • Home and care assessments if your condition has changed and you need new home adaptations or equipment
  • Providing emotional support and looking after your overall wellbeing

Rehabilitation support and reablement care is different for everyone, depending on your individual circumstances. If you're recovering from surgery you might only need care for a few weeks, whereas recovery from a stroke or loss of limb may take years.

Whatever your requirements, our person-centred approach means we can tailor our care to your individual needs, providing you with as much or as little help as you need to adjust to being home again and regain your independence.
